A penile prosthesis is a lasting solution that comes into play when erectile dysfunction does not respond to methods such as medication, shockwave or injections. Many question marks and misconceptions circulate about the topic; in this article we explain the process in a plain and honest way.
What is a penile prosthesis?
A penile prosthesis is an implant surgically placed inside the penis that provides an on-demand erection. Unlike medications, it brings a lasting mechanical solution to the problem. It is a proven method applied for many years, with high patient and partner satisfaction.
Two main types
- Malleable prosthesis: Two flexible rods placed inside the penis that can be shaped by hand. It is simple and easy to use.
- Inflatable (3-piece) prosthesis: Consists of two cylinders in the penis, a pump in the scrotum and a fluid reservoir in the abdominal area. When the pump is squeezed an erection is achieved, and when emptied the penis returns to its soft state. It is the type closest to natural and the most preferred.
Who is it suitable for?
A penile prosthesis is usually considered after other treatments have been tried. Suitable candidate groups:
- Those who do not benefit enough from medication (pills), shockwave (ESWT) or injection treatments
- Those with advanced erectile dysfunction due to diabetes
- Those who develop lasting erectile dysfunction after prostate cancer surgery
- Those with a serious erectile problem accompanying Peyronie's disease
- Those seeking a lasting and reliable solution
The surgical process
The operation is usually done through a small incision between the penis and the scrotum (penoscrotal approach). This way the incision scar is almost invisible. During the procedure, the prosthesis components are placed in anatomically correct positions. The operation usually takes a few hours and requires a short hospital stay.
Recovery and use
Swelling and mild pain in the first days after surgery are normal and are controlled with painkillers. Return to daily life is usually within a few weeks. Active use of the prosthesis begins after the tissues have fully healed — usually within 4–6 weeks — with the doctor's approval. A penile prosthesis does not make sexuality "artificial"; it only restores the lost function. Sensation, orgasm and ejaculation are largely preserved.
Realistic expectations
To be honest, the right expectation is half the success:
- Sensation and orgasm are preserved: The prosthesis only provides firmness; skin sensitivity, orgasm and (if present) ejaculation are not affected.
- Firmness is on demand: With an inflatable prosthesis you achieve an erection whenever you want, then return to the soft state.
- It is long-lasting: Modern prostheses work without problems for many years; they can be replaced if needed.
- Satisfaction is high: Both patient and partner satisfaction are quite high with the right candidate selection.
Are there risks?
Like every surgical procedure, a penile prosthesis has risks — the most important being infection, but with modern antibiotic-coated prostheses and sterile techniques this risk is quite low.
